What is this form in the Submission Log?

Hey Amelia,

Since you're very familiar with the forms here, I thought to ask you first
if you know what this form is shown in the HQ submission log.

http://dev.commcarehq.org/BRAC/CHP/HomeVisit

The only thing that I can think of is a newborn visit. Is there a way to
confirm this?

Hey Nick,

The only Dodoma form I can find with that namespace is the household
registration form. Let me know if the submission content doesnt make
sense in that context.

Yeah, hard to evaluate in the submission log but seems right.
