Reminders on commcare & SMS

Hi Guys,
I am trying to configure reminders on commcare to remind clients of their
next visit date.
I have two questions:
1.How do I set the client (case) phone number for the reminder
2.I see sms usage under the report section, what is the limit of free SMS
available on the platform, and how is the excess charges and at what rate?
