Need urgent support for unrecoverable error

I need urgent support.
Why, when I entered as a mobile worker to test case management registration form and entered cases, and at the time that I did sync server on the phone, and I went again to register another case, it refused and showed me this message : unrecoverable error this form affects the user case, but no user case id was found. please contact your supervisor.

P.S: I created a group for that mobile worker and the case sharing
