Multiple errors when submitting webforms

I am getting this error when submitting a record through a webform:

Can you please report an issue from your CommCareHQ project space and
include details with the app and form you are submitting as well as the
user you are submitting with.

Hi,

I'll email you off-thread with your ticket number. It might be that you may
need to check your spam folder / adjust your spam filters to allow the
emails through.
