Force close error with test application

Hi,
I was attempting to familiarize myself with Commcare by setting up the test
application as per the Beginner Tutorial on the wiki. I built the app, but
when I try to run it on an Android phone as a mobile user, the application
shuts down when trying to open the first form (Pregnancy--> Registration).

The error message says: "The application Commcare ODK
(process.org.commcare.dalvlk) has stopped unexpectedly. Please try again."
It happens every time I try to open the form and initiates a force close.

On the emulator, the application also does not work, though it stalls at a
different step (when trying to load the Follow up form for any case).

Let me know if anyone has encountered this or has any suggestions.

Which version of CommCare ODK are you using? Is this CommCare ODK 2.0
downloaded from the Google Play marketplace? Does the Registration form
work correctly in the emulator?

Yes, I downloaded Commcare ODK 2.0 from the Google Play marketplace.
The Registration form works fine in the emulator, but the Follow-up form
does not.

Sorry for the troubles here!

I ran your app on our upcoming build (2.2) and am happy to report that
instead of crashing, you get the error message "Location /data/tetanus1 was
not found", so it looks like there's either a missing node or a bad
reference in the form. Hopefully the newer version will make it easier to
debug these kinds of form problems in the future.
