Does anyone have a good set of focus group-style questions to ask CHWs about
their experience with CC, and any issues they may be encountering? I know
observation data is the best/most reliable (and the observation form on CC
is great, thanks Amelia and all who worked on it!), but I'm not sure how
many home visits I'll be able to observe on my first few trips out to see
sahiyyas. I've thrown together a list, but just curious if anyone's already
put something more formal together.
I created a survey for our CHVs in Dodoma as a part of our form improvement
process. This survey asks the CHVs how they feel about a specific form in
our application. This is our most general form and could be slightly
reworded for a broader context. We found this survey to be successful at
extracting useful information from the CHVs.
I've attached the survey form. Not sure if this is exactly "focus-group"
style, but you can check them out.

Added our BHOMA monthly survey that we ask them to fill in on their
phones to the wiki .

Cory - that's very interesting. Do you have any sense of how hard it was to
get them to submit this form and if they are good about doing it regularly?
The idea of monthly self-reported feedback is quite interesting.
As far as I know they are pretty good at filling it out every month.
They have monthly meetings with their supervisors and I think that
they fill out the form as part of the process (either before or during
the meeting, not sure which). Adding Linnea, Alex, Matt, and Ntazana,
who may be able to provide more clarification on exactly how it's
being used and how successfully it's been being filled out.
