Data Export from Form with case lists and details

Hi, I would like to export data from follow-up form including case list and case details. For example, I registered a patient in Registration Form and filled follow-up data in ANC form for this patient. When I export form data, the data does not include the patient's case lists and details. How can I solve this issue. Thank you.

I put the value from registration with hidden value in follow up form to export form data.
