Thanks for the recommendation.. I am already using a number of
single-select questions.. I have another 5 combined multple response
questions. So, in order to keep the survey with a reasonable number of
total questions its important that I apply this multiple response question.
Any help would be appreciated.

You can also add a multiple answer question to the form - to do this, in
the form builder click on the little arrow next to the single answer
question in the toolbar - this will let you choose a multiple answer
question.
