Case creation form not available

Hi guys,

I think the case creation form was archived but I do not understand why I
can not find it in the archived forms. The case is still open but does not
appear on the case list on HQ and on CommCare Mobile. I can see the other
forms that were submitted for this particular case but not the case
creation form. Here is the message I get when I click this case:
The case creation form could not be found. Usually this happens if the form
that created the case is archived but there are other forms that updated
the case. To fix this you can archive the other forms listed here.

The owner is mobile worker and not the group name. I would like to have
this case appear on the case list again, Any ideas? I am not sure I
understand that message clearly...

Thanks,
James

I have encountered same problem before.
My scenario was that cases were assigned to a different mobile users and
the original mobile user who created the cases were deleted. When a user
is deleted all forms associated with that user disappear too. In my
scenario I had ID of the deleted users and was able to restore them. This
caused the case creation form to appear again and the error disappeared.

In my situation, the mobile user is still actively submitting data on other
cases so I would imagine he was not deleted. All other forms for this case
appear in the case history but not this particular form. The case has
disappeared from the case list but still shows it's open when I look at the
case history.

Hey James,

Not sure if you have already tried this, but you can use the "Raw forms,
Errors, and Duplicates" report to view old archive submissions and
unarchive them if necessary. Just select only the "archived forms" filter
option.

Hi Cory,

Thanks for your suggestion. I tried that option but I still can not see the
screening form (case creation form) in the archived forms.

Thanks,
James
