I took a quick look at your app and it looks like you have an error in your
form. The 'feeling_sick' question is a multi-select but it has not options.
It looks like the two questions below that ('yes' and 'no') were supposed
to be options for the 'feeling_sick' question. If that is the case you can
add delete those and then add the choices to the select question by using
the 'Add Choice' button (it has a circle icon). You'll probably also want
to change that question to a single select instead of a multi-select. You
can do this by clicking on the question and then in the top left corner of
the 'Question Properties' section there is a button which allows you to
change the question type.
