Below is the normal screenshot that I get, but I just did an install on a
Samsung Galaxy SII (Model SGH-T989) on Android 4.1.2, and there is no menu
at all!

How do I get that menu back?

Does the device have a physical "menu" button? Some older phones supress
the visual "three dot" menu, and require you to use Android's old "menu"
button in hardware to pop up these types of menus.
