when i filled the entry on follow up form, does commcare create a new row for each home visit follow up or it just update the previous entry and keep the updated one. and if we want to see the history of home visit entry , can we do that..otherwise the report would not show any historical data

When you use a follow-up form to update a case property, that property gets overwritten. This means that every time you do a Case Data Export you will only be able to view the most recent values for each property.
However, each form entry is saved individually, and you can access historical form data by doing a Form Data Export.
You can also access case data in your browser by navigating to Reports -> Case List. You can access form data by navigating to Reports -> Submit History.
